Dear Friends,

As part of MOH’s efforts to improve palliative care for the terminally ill, the Ministry will be reviewing the Advance Medical Directive (AMD). An Advance Medical Directive (AMD) is a legal document that one signs in advance to inform the doctor that one does not want any extraordinary life-sustaining treatment to be used to prolong one’s life, in the event that one becomes terminally ill and unconscious.

An AMD is an important part of planning for the end-of-life process because timely discussion and planning will help ensure that the patient’s wishes are respected in a dignified and humane way. The public can make an AMD by filling in an AMD form, signed in the presence of two witnesses, and returning it to the Registrar of AMDs.

MOH is currently working on simplifying the form and will be organising a focus group discussion with REACH to find out how it can further simplify the form and hence the application process.
